Voluntary Life Insurance

VOLUNTARY LIFE and AD&D INSURANCE – Symetra

In addition to the County provided life insurance, employees may purchase additional life and AD&D coverage for themselves, their spouse and/or dependent children.

New hires and newly eligible (such as part-time to full-time):  Full-time employees are guaranteed coverage up to $150,000 (or 5x your salary, whichever is lower), and up to $50,000 for their spouse and/or $20,000 for their eligible* children.  Note: coverage for spouse and/or children cannot be more than the employee’s own coverage.

*Eligibility rules apply for dependent children: Ages 19-25 must be full-time students; not available over age 25 unless disabled dependent per IRS definition prior to age 18.

Employees have the option of applying for coverage above the $150,000 – up to $250,000 (but not more than 5x annual salary) for the employee; and up to $100,000 (but not more than employees coverage) for their spouse.  To apply for the additional coverage:  Submit a completed Evidence of Insurability Form (EOI) to the insurance company. The additional coverage will be added when/if the insurance company approves the EOI application.

IMPORTANT:  TO ELECT VOLUNTARY LIFE ENTER THE AMOUNT OF COVERAGE (NOT THE PAYROLL DEDUCTIONS)THE SYSTEM WILL CALCULATE YOUR PREMIUM BASED ON YOUR AGE.  The attached chart is just for reference:  RATE CHART

What is AD&D Insurance?  AD&D stands for Accidental Death & Dismemberment. AD&D coverage provides added protection against losses due to a non-work related accident.  Benefits vary depending upon the type of injury sustained.

NOTE: You may be able to Continue Your Coverage after your employment with Clermont County ends, by ‘porting’ your voluntary life coverage and/or ‘converting’ your county paid coverage.
