Health Savings Account

➢ This account is yours. You can continue to access funds for medical, dental and vision out of pocket expenses until funds are depleted.


Flexible Spending Accounts with Chard Snyder

➢ Health Care/Dependent Care – This account ends on your last day of employment. You have 90 days from your separation/retirement date to submit unpaid claims (incurred prior to your employment end date). After 90 days you will forfeit your balance.

Dental/Vision Coverage

➢ The P&A Group will mail a COBRA Notification explaining how to continue your dental and/or vision plan after your employment ends. You have 60 days to elect COBRA.

Voluntary Life Insurance Benefits

➢ Login to Employee Self Service (ESS) to access your payroll and W2 documents.

Sydney/Anthem Wellness Plan

➢ If you elected medical coverage, you are able to participate in the Wellness Program. If you earned a reward, but did not redeem it you will have 6 months to call the customer service number on the back of your ID card to request your redemption.


  • Note: you must redeem your rewards within your current medical plan year eligibility. Any rewards not redeemed during this time will rollover for 3 months into your next eligible plan year, after which they will disappear from your storefront.
